I've also experienced some funny behavior with BlinkMeNot, since the 2015-09-20 FFXI update.

Players I'm targeting will change models suddenly, examples:

         - Hume ally is suddenly a black chocobo

         - Hume ally is suddenly a yellow chocobo

         - Hume ally is suddenly a Tarutaru

         - Self (typically Mithra) is suddenly a Galka, gear equipped is gear I own, changing gear again reverts to normal Mithra model

         - Elvaan ally is suddenly a Mithra

         - Mithra ally is suddenly a Hume

I like to play with the BlinkMeNot hex setting bmn 85e

I've tried uninstalling Windower, deleting the Windower folder, and reinstalling Windower, no change.

unloading the plugin prevents it from happening but I miss this plugin a lot

BlinkMeNot has been deprecated and replaced my the AddOn DressUp. However, DressUp is currently having a few issues of its own right now.

BlinkMeNot should be working fine, I updated it and will keep updating it to work as it does now. I cannot reproduce any of the errors people are reporting with it, so fixing it is not that easy.

I find that BlinkMeNot won't work as intended if I have someone targeted I can lose focus on them somehow.  I believe that the model changing occurs during this lose focus.

I'm not sure if it's when I'm changing my gear casting a spell or they're changing gear, or what triggers it, but it's a daily occurrence.

Is there anything I can do when the issue happens, to gather info that might help you diagnose or reproduce?

I'll see if I can get someone in game to help me try and reproduce it consistently.

Last night among the normal "changing races randomly" when casting, targeting, and doing gearswaps with blinkmenot enabled... I also noticed that I changed into a Galka immediately after using a warp ring.  This is the first time I've seen it occur when using an item.

I turned into a Galka as the item was used, after zoning I was a Mithra again

Made a video of the behavior, loaded and unloaded blinkmenot to demonstrate it was only happening when bmn is loaded:
